What do the two numbers on your blood pressure monitor actually mean? The top number (systolic) measures the force when your heart contracts. The bottom number (diastolic) measures the pressure between beats. Together they place you on a seven-tier scale from hypotension to grade 3 hypertension. This calculator classifies your reading according to ESC/ESH guidelines, calculates pulse pressure and mean arterial pressure, and flags isolated systolic hypertension - the most common form in older adults that standard readings can mask.
The four key numbers
How to measure blood pressure correctly
- Sit quietly for 5 minutes before measuring. No caffeine, exercise or smoking for 30 minutes prior.
- Sit upright with your back supported, feet flat on the floor, legs uncrossed.
- Place the cuff on bare skin at heart level, about 2 cm above the elbow crease. The tube should run down the inner arm.
- Rest your arm on a table so the cuff is at the same height as your heart. A dangling arm inflates the reading by 10+ mmHg.
- Take two readings one minute apart. Use the average. If they differ by more than 10 mmHg, take a third reading.
- Record the time and context: morning readings tend to be 5-10 mmHg lower than afternoon ones. Note whether you used the left or right arm.
ESC/ESH vs AHA classification
European and American guidelines use slightly different thresholds. The table below compares the two systems side by side.

Key difference: AHA lowered the hypertension threshold to 130/80 in 2017, while ESC/ESH keeps it at 140/90. A reading of 135/87 is "high normal" in Europe but "stage 1 hypertension" in the US.
